The AMA and “Ethics of AI in Health Care”

Today’s offering discusses the newest “innovation” of the American Medical Association (AMA): a new Journal of the American Medical Association (JAMA) devoted to AI and health care. The new journal’s website is here: https://jamanetwork.com/channels/ai, “JAMA+ AI.” Please see the opening statement by founding editor of JAMA+ AI, Dr. Roy H. Perlis, MD, MSc, from the above-linked website:

The AMA has offered CME (Continuing Medical Education) courses for some time now that are related to the use of artificial intelligence / “large language models” / “machine learning” in healthcare, via its website (https://www.ama-assn.org/.) As of spring 2027, the new “JAMA+ AI” journal will officially begin. The incoming editor of the new journal, Dr. Roy H. Perlis, MD, MSc, was just named as the Director of the Bakar Computational Health Sciences Institute at the University of California, San Francisco. Dr. Perlis is already associated with Massachusetts General Hospital and the Harvard Medical School (https://medschool.ucsf.edu/news/announcing-roy-h-perlis-md-msc-director-bakar-computational-health-sciences-institute, 29 September 2026.)

However, there are already articles appearing on JAMA related to the new journal, to using AI in multiple areas of healthcare providing, and more. Three examples of such articles: first, https://jamanetwork.com/journals/jama/fullarticle/2855492, “Engaging With Medical Evidence in the Age of AI”, Rohan Khera, MD, MS, et al., 15 October 2026; second, https://jamanetwork.com/journals/jama/fullarticle/2855491, “Clinician Use of an Artificial Intelligence-Enabled Clinical Information Platform”, Vishal R. Patel, MD, MPH, et al., 15 October 2026; and, third, https://jamanetwork.com/journals/jama-health-forum/fullarticle/2855161, “Avoiding a 2-Tier AI Ecosystem in Health Care”, Brett Moran, MD, et al., 16 October 2026. Following are two screenshots from the article by Khera, et al.:

Note the mention of heavy (“disproportionate”) use by “newly-minted” physicians. Following are three screenshots from the Patel, et al., article:

Note, in the Table above, the four areas under subcategory “Clinical decision-making”; the five areas under the subcategory “Clinical knowledge”; and, the “Guidelines” area under the subcategory “Evidence.”

Finally, two screenshots from the Moran, et al., article:

All of the above bring up questions, among them:

How will Medical Doctors — Physician Assistants — Osteopaths (they are licensed as doctors of osteopathy [DOs]; they can diagnose, prescribe, and create treatment plans for patients, as do medical doctors [MDs]; DOs have the rights and privileges that MDs enjoy) — Nurse Practitioners, and other licensed healthcare professionals who diagnose, prescribe, order tests, and create treatment plans — be trained to use AI-generated information as an adjunct to their work, as opposed to using AI-generated information as the template basis of their work?

What about the proposal, outlined above in the Moran, et al., article, regarding the implementation of a “government-wide AI watchdog and information-sharing platform” under the control of CMS, for example?

Can such an “AI watchdog and information-sharing platform” become a “one size fits all” approach to patient encounters and patient care? If so, what does this do to the individual healthcare provider’s ability to, for example, prescribe a drug for an “off-label” use to treat a particular patient?

What would such an “AI watchdog” framework do to care provided in hospital or nursing home settings?

>>>>>>>>>>>>>>>>>>>>

The use of AI-generated systems for use in healthcare settings is increasing rapidly over the entire spectrum of medicine. It is Yours Truly’s opinion that, while AI-generated information systems may eventually become a tool to assist the provider in assessing and treating a patient, they should not be used as a template for a “one size fits all, follow the steps outlined on the AI-generated program and stay with that” approach.

I agree! AI/SI should not be in the driver’s seat on diagnosis.

And yet, With the right kind of guidance AlterAI.systems has been a tremendous aid to me personally for exploring beyond my own knowledge in the wellness domain. BUT you have to fight to keep it “tight” … even reminding it that you don’t want “wall of text” responses so it’s a back and forth.

If I wasn’t so well read already I doubt that I would gain the same level of quality out of the chats.

I agree. I don’t know whether a “one size fits all” approach is ever universally appropriate, and that definitely would not be the case with AI/SI. I’ve seen that AI/SI can be given parameters within which to work, so maybe the factors specific to each patient could be considered. But a template that fits everyone? No.

This thread by DataRepublican includes a 5-minute video illustrating what is said in the text and the tactics being used.

THE ILLEGITIMACY PLAYBOOK / THE DEMOCRACY FILES: A PRIMER

I will be using this thread to track all drops, so bookmark it.

I spent most of yesterday backing everything (now 312 gigabytes) up in various sources and getting the materials out to others. I also produced an AI video (below) to explain the whole thing to others are not on 𝕏 – feel free to distribute.

Here’s the primer:

USAID, the foreign aid agency, had a democracy office. Inside the agency, it was called DRG.

Officially, DRG administered “democracy assistance.” They’d go inside countries they thought were sliding into authoritarianism and help the opposition there. That assistance included election monitoring, judicary support, independent media, and civil society building.

You may have heard the term “color revolution.” The model comes from Serbia in 2000, where trained student activists brought down Milosevic with the help of Western governments, after sustained protests. With the help of USAID’s democracy office, this model was implemented in many other countries to topple governments: Georgia’s “Rose Revolution” in 2003, Ukraine’s “Orange Revolution” in 2004, and many more.

Then last year, DOGE shut down USAID. But the USAID democracy office didn’t stop. They formed a group called the Democracy Renewal Group, the same initials as their office (USAID Democracy, Human Rights, Governance). Their goal: apply their own playbook to the United States itself.

This goal became a plan, called Renew American Democracy (RAD). It invoked the “4 B’s” framework. Block – use lawfare and protests to make life harder for the Trump administration. Bridge & Belong – control dialogue and win people over. The final and most important B is Build. Build means redesigning the American government itself, and the crisis is the opening.

In their own words:

“We can take that breakdown moment and leverage it for a big structural reform, you know, refounding moment.” – Danielle Reiff, DRG lead.

Calls show DRG working alongside DSA on ideas of creating a different government, one that does away with the Constitution itself.

DRG does not run the Block operations. They hand it to UDA, the United Democracy Alliance. One organizer calls it a “huge galaxy”: lawsuits, street protests, non-cooperation. They list No Kings, Indivisible, MoveOn, 50501, and many other mega-activist NGOs among their members.

Another organizer calls UDA Erica Chenoweth’s project. Chenoweth is the Harvard professor behind the three-and-half-percent rule of regime change: get that share of a country’s populace onto the streets, and historically, the movement wins.

The UDA is very “hush-hush.” They have no website, no roster, no filings.

Then come the red lines: things the Trump administration could do, such as calling the Insurrection Act on protesters, that would mean that the federal government, in their words, “crosses the threshold into illegitimacy.” Nobody voted on those lines. There was no public debate. These red lines were developed within secretive working groups, and in a paper addressed to governors and staff.

In the words of Reiff/DRG:

“So that if there is a red line crossed in terms of a Constitutional crisis, like everyone’s already pre-organized. And so Mike Smith actually said, he goes, oh, you guys have the day after tomorrow plan.”

Cross a line, and the paper’s options include states refusing to lend their resources to federal officials, and governors convening what it calls a “Third Continential Congress.”

Running the UDA’s Insurrection Act working group: Rear Admiral Mike Smith (RET) of National Security Leaders of America (NSL4A). NSL4A is also credited with operationalizing UDA, the blocking operations. NSL4A consists of about 1,600 ex-officers, spies, diplomats. They’re turning red lines into indicators and running a situation room to monitor them throughout the midterms and beyond.

The NSL4A’s lawyers told them to stop recording meetings in case of subpoenas. But the rest of the network continued to record. DRG works in conjunction with Renew to Thrive (R2T), which ended up accidentally publishing 400+ calls and thousands of documents. These calls and documents were never meant for our eyes, but it’s them, on tape.

Thousands of NGOs and billions of dollars are involved in this endeavor. Most of them believe they’re saving democracy. The problem is that people who spent their careers deciding when other countries’ governments had lost their legitimacy wrote that test for ours, in private, and planned for what comes after.

Who decides when your government is legitimate?

Gudthots

It is sick that our government has been doing this all over the world with our tax dollars for far too long. Where in the Constitution is the government authorized to subversively destroy other countries?

Supreme Court Says New Hampshire Can Ban Voters from Using Student IDs at Polls in Midterms – Ketanji Brown Jackson Dissents

The Supreme Court on Thursday ruled 7-1 that New Hampshire can indeed stop voters from using student IDs at polls during the midterm elections.

The high court paused a lower court’s ruling that allowed student IDs to be using as a form of voter identification.

Liberal Justice Ketanji Brown Jackson dissented.

“I respectfully dissent because—even, and perhaps especially, when an election is looming—I would not disturb a clearly justifiable federal-court order issued to ensure that the public receives accurate information about a State’s electoral processes,” Ketanji Brown Jackson wrote in her dissent…

A podcaster named Jennifer Welch.

TheseTruths

Nolte: Podcaster Jennifer Welch Demands ‘Pistols Firing’ at ‘Antichrist’ JD Vance

Jennifer Welch, a left-wing podcaster who has interviewed the biggest names in the Democrat Party, is demanding that there be “pistols firing” at “antichrist” JD Vance to ensure he does not become president in two years.

In a broadcast that is sure to (and should) get the attention of the Secret Service, Welch vomited a pile of hate and violence at our current vice president that only a Democrat could get away with. If a Republican said something like this about Kamala Harris, he or she would be run out of politics.

Keep in mind that Welch is not some fringe podcaster. She is firmly embedded and welcomed in the Democrat Party.

…And here’s this very same Jennifer Welch targeting our current vice president for assassination, and using two methods, signaling her desire to see him murdered. First, she dehumanizes him as a “white supremacist,” the “Antichrist,” and a “sociopath” who is “not fit to be a father.”

Second, she declaratively calls for “pistols firing at all time [sic] towards this guy.”

This is what Democrats do, and they do it with the full support of the corporate media.

Welch appears to be desperately calling for some mentally unstable leftist to murder JD Vance, to assassinate the sitting vice president, to gun down a husband and father of four, and to do it before it’s too late, before the “Antichrist” becomes president.

…Democrats are no longer hiding their true intentions. Instead, they are hoping we don’t believe them.

Believe them.
