For the time being, given our national emergency, we are switching to DAILY COVID-19/China Virus threads instead of weekly threads. Your comments and updates are most welcome. PLEASE continue to post your valuable information, hard source links, and local observations within these Daily Threads. Additionally, we will try to include links to government and official pressers, so those who no longer have cable can remain informed.

We’ll get through this crisis, together. As a group, there is probably no one better prepared than we are.

We started accumulating information about this virus in mid-late January, in what became almost weekly threads. Update 3/26/20: Neil Ferguson, father of the model discussed in this article, has now walked back his original prediction of 500,000 deaths in the UK—just one day after the nation went on lockdown. Before a full analysis of the effects of the policy can be evaluated, he now predicts 20,000 deaths or less and that U.K. hospitals will be alright.
Local governments across Texas recently enacted their next step in attempting to stop the spread of the Chinese coronavirus: limiting business activity and requiring people to stay at home except for essential needs. At least one county judge based his decision on a flawed study, bringing into question if “shelter in place”—and the methods by which it’s being enacted—is the appropriate response.
Dallas County Judge Clay Jenkins’ “shelter-in-place” order was made without discussion or consent from the rest of the Dallas County Commissioners Court. An emergency meeting was held Monday, and the first question—from none other than Democrat Commissioner John Wiley Price—was: why?
Jenkins touted data from a model called Covid Act Now, a model that’s spread around the world, even making the rounds in Harris County. According to Covid Act Now, Texas will likely experience a fatality number of 430,000 if we don’t “shelter in place.”
But this model comes with some asterisks—called “known limitations.” . . .
. . . The die is cast. Let’s hope that when this is over, we won’t be having funerals for small businesses in Dallas County—and everywhere else that “shelter in place” has been enacted.
The only defender of what little freedom of enterprise remained in this blue county was infamous Democrat John Wiley Price. Our world is truly upside down.

People: Please, Please stop dividing the current # of deaths into the current # of known cases and calling it the death rate. That is NOT how the death rate is calculated. That is fake news, even if it is not on purpose. We need to be responsible in fighting fake news, not creating more of it ourselves!
When we don’t have our basic facts straight we don’t understand decisions that are being made and we do not sound informed when we are speaking to Dems/leftists on social media or in person. Facts matter.
One more time…
If we have 100 cases of the WuFlu and 10 people have died and 10 people have recovered… we do NOT have a 10% death rate!
10% of the known, documented via testing cases have died but that is not the death rate.
Why? because the 80 cases that are left are Not yet resolved in death or recovery. We do Not yet know the death rate of group A until all 100 cases are resolved in death/recovery.
At that point we can say, 50 people died and 50 people recovered which gives us a 50% death rate.
We do NOT calculate the death rate by dividing those who have died into the total number of current, ongoing cases.
Now, the death rate will be less than the percentage of resolved, diagnosed cases that died. Why? Because we Know that there are more cases than those that are documented – same as the flu.
Over time, as stats become known, the CDC will make an educated guess/estimate of how many undocumented cases we had – the # of none symptomatic cases, the number of mild cases and the number of harder cases who still didn’t get tested &/or hospitalized. Then they will add those #s of all estimated/projected cases to the known cases divide that number by all of the estimated deaths (documented + an informed estimate of the people they figure died due to the WuFlu but were undocumented/tested). That will be the death rate.
As things are developing they are studying the data and developing estimates as we speak.
However, the death rate is NOT deaths divided into known cases. Stop doing that as it makes MAGA supporters look silly.

currently of US documented cases of Wuflu we have had 1,300 deaths/1,868 recovered….of 3,168 resolved cases 1,300 have ended in death.
That would look like a disastrous ratio/death rate except:
1) death seems to be quickly by days and even weeks than recovery so of those who got sick a the same time those who would pass seem to do so after a few days but actual, full recovery takes multiple weeks.
2) there are many more cases that have had WuFlu and recovered that are undocumented via testing which account for Many more recovered
3) and we projected the death rate of those hospitalized with complications will also go down as more effective therapies are found
Remember, our current stats are a fluid situation of a Developing story. Everyday’s stats will increase for the foreseeable future, although hopefully at a reduced rate and then at manageable numbers (Not just the rate but also the raw numbers of increase once the total case load reaches a large total, right Steve? that is one of the points I got from your article) and every day is 1 day snapshot of a growing #, not a final total.

It is not my personal opinion so nothing for me to approve/disapprove…which is an odd way to post your question if you don’t understand the difference…but I am happy to help you understand.
AGAIN – they are not posting Death “rate” but the % of known cases who have died … which is not the actual death rate because the known cases include ongoing/unresolved cases. The above linked article helps explain this.
GA web site literally does NOT call that the death “rate”. BIG different. Words matter. This is not my “opinion”…see the info from your link:

They are noting % of deaths of known current cases, not death rate. Remember, that the rest of recovered/deaths will come down
1) as the numbers of informed estimates of the mild cases are added into the numbers, just as the CDC does every year for the flu. This will greatly increase the estimated WuFlu totals but will reduce the apparent death rate.
2) and as more cases are resolved – recoveries take weeks and for hospitalizations for often for multiple days to weeks in the ICU.
As conservatives we want to be accurate and informed, more so than the average and facts matter. Some do calculate the death rate by dividing past deaths into ongoing cases…but that is not correct.

I care about rate of growth, # of new cases, complications rate and hospital load esp. on ICUs.
I also care about treatment options and the messing around with the solutions on the table is despicable! And the % of Americans who have it and are unable to work at any given time is another important stat to me.
But death rate is important as it is being misquoted too low and too high, partly in honest mistakes, partly (by some) as spin.
Its a number, a fact, not an opinion and some in the media are using it to bolster their narratives. we must correct the fake news but we have to do so correctly to be effective.
So many other aspects to this situation and you are right – Where did it come from, by who, how, why…All critically important! Thinking about that aspect of this deal Now is good so that you gather while it is still fresh and the narrative is not 100% streamlined…and things are accidentally reported and/or still available online.
Also, the horrible way the Dems and media are politicizing this and creating fake news about President Trump, false narratives, etc. is just despicable!
the economy, long term impacts on society, long term changes to deal with other bioweapons for national security purposes, short term/long term impacts on us as individuals of the shut down and finally, taking care of our and our family’s health…so many important layers and aspects to thinking about and death with this situation!

How many pieces of fruit. 2 apple + 2 orange = 4 pieces of fruit.
Compute death rate. A (one or two words) divided by B (one or two words) = C death rate.
If it can be this simple, it would help slow folk like me.

More of a reflection on those who refuse to learn than the one who has earnestly and repeatedly tried to explain for days and days.
Impossible for me to understand wanting to stay ignorant v. learn. When Steve posted his article on exponential growth, I studied it and learned that it is not just exponential growth but getting to a certain size too quickly because at that point, even if the growth rate flattens, it is still a large case load for the hospitals to carry and additional growth will still be larger numbers even if percentage is low.
I read that article multiple times because I wanted to Learn and Understand. Odd to me to run into people who actively don’t want to learn and are offended by facts.

If dealing in reality and facts raises one’s fears then one are not dealing in reality. One needs be fearful of facts. Facts don’t care about your feelings and truth has no agenda – don’t we want facts, regardless of some people’s emotional reactions to the realities of a tough situation?

If we understand facts, we can deal in reality and then focus overcoming and achieving. If we do not understand the facts then we not make quality decisions or support those who are making quality decisions.
Hiding from facts (such as constantly focusing on today’s numbers and asking why the strong actions taken by President Trump based on today’s numbers) is undermining President Trump and his leadership as well as undermining the steps he is trying to take to save American lives.
Comfort porn is just as bad as fear porn – both are inaccurate. Hiding from reality is silly, inaccurate and lessens the credibility of the person doing so and this blog regardless of how much better it feels and how much more comfortable it leaves one.
If someone comes here and reads inaccuracies, complaints and disagreement with President Trump’s recommendation to stay home, with statements ignoring the growth rates, etc….all of the blog looks silly and the posting we are doing on other subjects looses credibility.

Something to remember is that state health dept web sites, the CDC and the WHO dashboard are all slightly behind/not reporting in real time. I find the link below to be more accurate although I check the TN health dpt. web site as well as FL, VA, GA, CA, LA, NY and WA #s on a daily basis (hot spots and/or friends/relative’s locations) to get a detailed picture of what is happening in those locations.

We have had 53 recover. So we have ~1,050 active, known cases as of right now as 56 have been resolved/concluded in death/recovery. We know we have unreported cases and cases yet to be diagnosed but already infected. same as everywhere else.
We have had somewhat aggressive testing and early lock down of schools as soon as the infections were known and I think that has helped slow the spread significantly. the first case in Williamson County had children in local schools and son was found to be infected. Williamson County and the directly affected private schools shut down very early.
the testing of people connected to this family and in the schools helped find cases that were mild and that helped slow the spread + has given us a number of recovered cases despite a relatively short time of having infections in our state.

We have a less than 10% positive rate of testing and, yes, 14K+ have been tested. We have had multiple testing sites around Nash and Williamson county (and I assumed Memphis as well). Schools closed quickly infected counties and the whole state shut down schools within a short time. Staying at home is helping slow the growth.
Hospitalizations are less than 10% of known cases…again, because we are catching a lot of cases early and most of our diagnosed cases are 50 yrs and younger.
It came to TN via college students and traveling professionals. Williamson County case was from the BioGen conference/traveling in Boston, from my understanding. And it sadly got into our young people. We have almost as many 0-10 yrs as we do 70+ yrs. 🙁 Don’t like seeing children sick!

Oh…missed that completely.
Saw the post about TN #s, instantly knew it was way off, so shared accurately and talked about TN, what was happening here and so forth. Did so because a) didn’t want to let such errors stand for others reading down the thread to become misinformed and 2) hat that was the topic and the interest.
After posting TN data, stats and chatting about TN, I was curious about the rest of the comment. Thought maybe he was a media star and it was a well known quote or something. Weather has been was gorgeous yesterday and we were out gardening and beginning our spring haying. Thought maybe it was a Tennessean talking about getting out in the fresh air.
I had No idea it was snark. If it was…would have not have waisted my time with foolishness. Would have just corrected the gross error with links and moved on.
Was the suggestion for me to go back to school also snark? I took it as a compliment at the time. Happy being a housewife and due to husband’s MD/PhD educational background knew what that would actually mean and know I am way to old for all of that but still took it as a compliment.

Lol, it was meant to get outside and relax! Todd is a character in the movie “My Blue Heaven”. When he finally accepts he is in a neighborhood and not NYC, he is in his new yard, mowing, and laughing. The girls and I use the line often when we want a break to head outside. Not meant snarky. I have commented on, with much back up data and links to articles explaining, the following:
-proper calculations of death rates are important or we would right now think we had a Much larger death rate than we do (85K cases and 1.3K deaths…that would be a high death rate! but so would misunderstanding the current 1.3K deaths/1.8K recovered)
-making wild statements of 14K cases in TN
-saying that 1/2 of winter hospitalizations in VA are due to seasonal illnesses like flu and misunderstanding flu #s
-others usage of flu numbers … final flu #s include many assumed cases that are not yet built into WuFlu #s. We need to compare apples to apples v. to oranges.
-frequent misunderstanding that all of these reaction is about whatever the current day’s # of cases/deaths from Wuflu happens to be and not understanding that President Trump is making decisions based on where the numbers are going over the next days and weeks. And we need to be aware of what the real issues are v. today’s #s.
I have learned much on all of the above (esp. methodology for surveying flu activity in US) and shared.

Oh yeah – I am well aware of the trap. Nobody is “falling for it” here. But the reason the trap works is that, at the bottom, there is real bait.
What it DOES in fact do, is prove that the opinion (whatever it is) is not merely an outlier, as the other side will allege, but has some actual adherents, willing to go public. There are real people – those signed on – who believe it. At that point it becomes fluffy social influence, but “let all compute what they will”. It’s a form of news.
THE DATA is real. What it means is up for grabs.
One of the best examples was “50 IC big names against Trump”, followed by “60 military big names for Trump”.
The anti-Trump IC letter, and the pro-Trump MIL letter, were EXTREMELY instructive, in terms of WHO believed WHAT. The exact numbers were not so useful, except in the context of the order.
IC struck, and .mil struck BACK. A pattern, as it turned out.

Here are further notes from Michigan, as I’ve heard/seen so far.
My husband does purchasing at his company for that location & his boss said it was up to him to keep parts coming in to justify keeping employees on the job & not in lay-off. A co-worker went on voluntary lay-off.
Daughter worked her last restaurant kitchen prep shift til likely mid April. She was still given an “essential industry” letter from her manager, perhaps in case she needs to come in (she’s going to keep it in her truck whenever she needs to be on the road, just in case). She said they went from 8 people on a shift to 4 & now to 3. Her work can now be done by a manager apparently. Daughter was already doing the work of a dish tank along with her usual KP duties. This is an eat-in restaurant that did carry out prior to CV issues. They have been using Door Dash for deliveries now too.
Heard on radio the new Chick-Fil-A has created a whole delivery system. They are also offering free food to health care workers.
Serendipitously, daughter just finished training to be a “staff” for her disabled brother. Just like God to provide a way to (partially) make a living working at home & w/ a needy family member. They did their first shift together today & it went very well. There will be many creative options for them even in a Lockdown.
Son’s friend works in a top quality elder care facility w/ all levels of care. He is mostly working from home to protect their residents from contamination as much as possible. He recently moved out of his parents’ (really the church parsonage) home into an apartment & is dealing with a lot of isolation. His parents’ plans to return to the mission field may be delayed by all the CV upheaval, as likely will be the church’s search for a new pastor…
Another friend was pulled over by a cop yesterday & given a $500 ticket for being on the streets without being able to justify his outing (as in not going to/from work or for food). Not sure what community, but this seems excessive.
Husband’s band was going to practice, as usual, in the husband/wife house (guitar/singer) so just drummer & bass (hubs) would travel. As it was the day of the governor’s “stay home stay safe order” they decided not to risk it. At least one planned concert/gig has been postponed or perhaps cancelled.
Husband’s church has already decided to cancel their Easter service, & he’s bummed for he was to play bass…
On a store run to Sam’s club quite a few things were out including Chicken, Ground Beef, & a lot of fruits & veggies, I believe. There was still a decent amount of pork. It wasn’t busy but more like a “ghost town”.
Yesterday 2 sons threw football in street in front of the house & also got dog on a couple of walks. Today kids went to a local park to kick a soccer ball around for fresh air & to get into the community (goals on disabled son’s IPOC, Individual Plan of Service), some thought even being out in our own backyard was illegal…are we under martial law now or do we still have at least some basic freedoms???
Public library is closed til early/mid April depending on developments & nothing is supposed to be due/overdue in that time but an item due “today” couldn’t be renewed so we dropped it off just in case.
Son had the video chat Bible study w/ a few guys last night & though one of the free video chatting services kicked them off part way through they ended up “meeting” for about 1 1/2 hours. He also bought a video gaming system & online service so may play some games remotely with friends, something his older brother can coach him in.
We continue to get mail & Amazon Prime deliveries without issue.
Spoke with parents (79 & 80) & they are doing well at home, though Mom is getting stir crazy. They got to watch church on TV & were even playing cards to pass some time. They don’t want any of us to visit though we’ve offered any help they might need including shopping &/or other errands.
People are going a bit stir crazy here too since this is the time of year we start to come out of hibernation & those with forms of “Seasonal Affective Disorder” really want the doldrums of winter to be gone. Being cooped up doesn’t help when you are inherently “space” people!
Husband has commented that travelling about locally feels strange, like far fewer people are on the roads.
Daughter’s college is likely going to finish the semester online only. She doesn’t know how this might impact her plans to transfer to another local university for the Fall.
Heard on the radio, Rush’s show I think, about a small town in PA (Bloomsburg/bury?) that has all radio stations play the same song at a specific time in the evening & “everybody” goes out on their front porch & dances. It’s supposedly gone viral online & is a big encouragement to the community.
A Detroit police officer died of CV recently (& he was in his 30’s I think) so now all cops will be fever checked before shifts.
Husband’s work is leaving doors open, less touching of door handles, so he hears all the noise from the shop floor (very Loud industrial work) & freezes his butt off all shift. Some co-workers are very paranoid about the disease & work hard at distancing.
Made a crock pot version of 15 Bean Soup from pork chop scraps & stuff around home, practically a free meal, hearty & filling & reasonably well received.
Recently loaded special needs son’s 2 week pill box & added Vitamen C tablets to his morning & evening pill regimen to help him keep up his immunity (he’s the post liver transplant guy on anti-rejection meds). Oh he also asked his girlfriend to marry him, & she said yes, so when all this CV stuff dies down it will be time for some interesting meetings w/ her parents/Mom (her dad is autistic) as she also has special needs, so putting on a wedding, let alone a marriage, between 2 special needs people will be a whole new level of a learning curve–Whee!
A caller to Guy Gordon’s radio show on WJR tried to discuss Dr. Fauci being a “deep stater” & GG cut him off & said he wasn’t going to “allow anyone to peddle that crap around here”. GG was apparently doing his show from home because he was fighting an illness so protecting co-workers.
Tried to watch Trump team’s press briefing today (we don’t have cable) so started on Fox/2 which was showing Fox News’ version, they cut off before Q & A so went to NBC/4 & they cut off about the same time, then to ABC/7 & they went just a bit longer & had discussion w/ Stephanopoulis so went to CBS/62 & they carried some Q & A but when Pence was coming to the mic they ended, PBS channels didn’t carry it at all…

Ironically my undergrad degree was pre-med but I got so burned out on science (which I love) that I wanted to take a side jaunt in another arena & “have my own life”…so just a few years later I get “the most complicated kid” (ever seen in a number of medical practices) ultimately have “no life” of my own any more & have to develop near MD level of specialized knowledge on his various issues…of course with no pay & practically no sleep!

Well, since you mentioned it, my son actually said he proposed in a text by sending a GIF of some sort that said “will you marry me” & tried to then unsend it. But she said you can’t unsend it because “I already said yes!”
How hilariously autistic, at least the way autism presents in my son!!!
What’s interesting is that since they both have special needs they presumably won’t be able to be financially independent, so we’re anticipating they will likely need to live with one family or another. Currently our son is on SSI (& has been virtually his entire life) & he has been working 3 hours/wk for a few months (currently on a CV hiatus) & supposed to bump up to 2-3hr/week shifts…this is no where near sufficient funding to support himself, let alone a wife.
We don’t think she’s ever worked & we don’t think she’s had “services” like our son does (w/ respite care & “community living supports” staffing). Now that our daughter can be a staff for our son IF her family decides to pursue that type of support for her perhaps our daughter could assist her too, maybe making such a transition easier & giving the families a way to be more on top of the needs they may have as a couple going forward.
It’s going to be so interesting, any marriage scenario is, but this one is likely more complex than most. There is a multi-ethnic side to her family so we may end up tripping over each other culturally accidentally. Thankfully we’re all strong Christians so we’re just talking about uniting 2 different branches of God’s family! What a huge blessing. Just like God, making love out of nothing at all! No one could have guessed from our family constellation that the second kid to approach marriage would be our very complicated special needs son!

Our son talked with her parents & “asked” them (or at least the mom) before proposing…but he never had those talks with Us beforehand. However, given the nature of his conversations recently it certainly clearly was on his mind.
He’d actually bought her a ring, just to give to her (since he’s had an income & a girlfriend we were amazed to discover just how much of a “giver” he is)…but maybe in his mind he wanted it for engagement (which he actually kept to himself–something which almost Never happens in any domain with him)…
Anyway, with the CV restrictions around here they haven’t been able to see each other in person very much. The other day they (girlfriend driven by her mother) stopped by our driveway to give him a cloth mask they’d made for him & he gave her the ring without fanfare…I think they might be using it for an engagement ring because she loves it so much…interesting times.

Scenario: Trump overreaches his power, and is taken to court by the governors:
* TRUMP WINS IN COURT: Basically, this TRASHES the 10th amendment and rips it from the Constitution. The leftists lose a short term battle but win a long term strategic WAR objective
* TRUMP LOSES IN COURT: Kneecaps Trump badly and sets up path to impeachment. Media portrays him as the authoritarian dictator. They will either get him in the Senate or get him in the Nov 3rd election. Barring that, they will use the court case as precedent to challenge and tie him up in court on everything else that they can – ironically, the LEFT will be leading the charge against the Executive branch overreach!
Either way, the OVERREACH scenario is a win for leftists for another key objective: MORE CONTROL, more power centralized. They may loath Trump but they don’t loath power – they’re just mad that it isn’t their guy in the White House, period.
They weren’t quite prepared for the scenario where TRUMP DOES NOT OVERREACH. While it was a logical possibility, I cannot think of a SINGLE situation in U.S. history where there is an existing precedent for NOT overreaching during a crisis. Maybe someone can assist.
However, they do have a blue-left alliance of governors who are ready to create trouble for the President and blow it back on him for NOT DOING enough.
Scenario: Trump does not overreach his power
* GOVERNORS BLAME TRUMP: This is the origin of the “Trump’s Katrina” or “Trump’s Chernobyl” narrative, that started even before the China Virus issue was really on the upswing. The idea is that “Trump is not doing enough to help the states”. Another win/win for them: Trump either abuses power or he fails the states.
* GOVERNORS WORK WITH TRUMP: They were NOT prepared for this scenario. They have no plan in place for this. Trump has given the governors exactly what they’ve asked for. He’s seized the initiative to be proactive with them, highly engaged, and advances constantly in terms of problem solving. Even Newsom and Cuomo are knocked back on their heels. Cuomo tried to trap Trump on ventilators and got swatted big time. Newsom will try something similar but right now he is stuck in OWNING the mess they’ve made in Cali and if he has a real problem, Trump will be out in front to solve it. Radical public transparency has made it impossible to defeat the Trump.
It’s like the 1980 USA hockey team. Trump has taken their game and shoved it back in their face. The Soviets have been the best team in the world and they have no idea what to do. THEY LOSE.
The blue-left political establishment is now grabbing at anything they can think of to stall what’s coming via state/local actions:
* Unconstitutional power grabs
* Shelter-in-place orders
* Policing and Big-Brother surveillance
* Orders blocking therapeutics and medical assistance – will NOT go well with voters
* Trying to KILL small business and local economics – ECONOMIC KAMIKAZES

* STREEEEETCH out the length and duration of lock down orders as long as possible . . . for the economic phase, since political weaponization failed . . .
That leaves their next hope: Economic Sabotage (which I laid out in a separate post)
* Small businesses, especially restaurants and retail, take a huge hit and die
* Massive ILLIQUIDITY, huge economic destruction, toxic financial contagion
* People laid off, furlowed – huge job losses, massive unemployment claims
* CEOs and BIG BUSINESSES intentionally sabotaging their own companies to build narrative
* Media “reporting” on the massive financial crisis

Ironically, as you’ve likely seen/heard in the news today is that Michigan is now one of 3 new states claimed to be CV hotspots. I hadn’t heard about MI limiting the quinine options. Do you have a source for that info?

As of yesterday around here, and we’re just going to talk about the little island in the middle of the Metro Area which is only about a third of the population, and a tenth or so of the land mass in two states, these were the numbers I found.
Population STL City & County: roughly 1.2 million
Number of cases between the two: 230 – this may change today
Percentage of population with the bug: .2% ish
Now, that doesn’t include the outlying areas ans surrounding suburban counties where there are no universities, people don’t travel nearly as often, and where the suburbs expanded, but, um, only two people have died. Kansas City’s numbers are a similar rate for a smaller metro (YES! It’s smaller and their barbecue sucks.)
Either we really did flatten the curve with the stay at home orders, or it was massive overkill.

Daughn – here in CO there are NO textbooks. Two g’kids from kindergarten through 10th grade and I have yet to see one textbook. Yes the kids lug around gigantic, heavy backpacks, but they are loaded with spiral notebooks, folders, and planning calendars 🙄 – one per class?
That is how the schools keep us removed from oversight; we have no resources to try to understand what is being taught. At best, the kids bring home 1-2 pieces of paper with problems to work out their convoluted math or to solve word problems that ALWAYS are about Juan & Miguel, the poor migrants, … OR their homework is online multiple choice or googling something.
It has been very difficult to have any input into their learning because we don’t have access to the TEACHING part of the lessons, only the homework sheets. Very very frustrating!

Interesting there are no textbooks while kids are under parental oversight, but they get to college where textbooks are an enormous cost; the textbooks are revised annually so used book $$ savings isn’t possible, and the textbook authors are the professors. It’s a real early-indoctrination K-12 followed by huge money-grubbing debt-burden for college students.

I’m a reddit noob so I don’t know how to verify this to the sub rules…but I also found it interesting. Here’s the text-
Repost from another group. Interesting read.
“I am an ER MD in New Orleans. Class of 98. Every one of my colleagues have now seen several hundred Covid 19 patients and this is what I think I know.
Clinical course is predictable. 2-11 days after exposure (day 5 on average) flu like symptoms start. Common are fever, headache, dry cough, myalgias(back pain), nausea without vomiting, abdominal discomfort with some diarrhea, loss of smell, anorexia, fatigue.
Day 5 of symptoms- increased SOB, and bilateral viral pneumonia from direct viral damage to lung parenchyma.
Day 10- Cytokine storm leading to acute ARDS and multiorgan failure. You can literally watch it happen in a matter of hours.
81% mild symptoms, 14% severe symptoms requiring hospitalization, 5% critical.
Patient presentation is varied. Patients are coming in hypoxic (even 75%) without dyspnea. I have seen Covid patients present with encephalopathy, renal failure from dehydration, DKA. I have seen the bilateral interstitial pneumonia on the xray of the asymptomatic shoulder dislocation or on the CT’s of the (respiratory) asymptomatic polytrauma patient. Essentially if they are in my ER, they have it. Seen three positive flu swabs in 2 weeks and all three had Covid 19 as well. Somehow this ***** has told all other disease processes to get out of town.
China reported 15% cardiac involvement. I have seen covid 19 patients present with myocarditis, pericarditis, new onset CHF and new onset atrial fibrillation. I still order a troponin, but no cardiologist will treat no matter what the number in a suspected Covid 19 patient. Even our non covid 19 STEMIs at all of our facilities are getting TPA in the ED and rescue PCI at 60 minutes only if TPA fails.
Diagnostic CXR- bilateral interstitial pneumonia (anecdotally starts most often in the RLL so bilateral on CXR is not required). The hypoxia does not correlate with the CXR findings. Their lungs do not sound bad. Keep your stethoscope in your pocket and evaluate with your eyes and pulse ox.
Labs- WBC low, Lymphocytes low, platelets lower then their normal, Procalcitonin normal in 95% CRP and Ferritin elevated most often. CPK, D-Dimer, LDH, Alk Phos/AST/ALT commonly elevated. Notice D-Dimer- I would be very careful about CT PE these patients for their hypoxia. The patients receiving IV contrast are going into renal failure and on the vent sooner.
Basically, if you have a bilateral pneumonia with normal to low WBC, lymphopenia, normal procalcitonin, elevated CRP and ferritin- you have covid-19 and do not need a nasal swab to tell you that.
A ratio of absolute neutrophil count to absolute lymphocyte count greater than 3.5 may be the highest predictor of poor outcome. the UK is automatically intubating these patients for expected outcomes regardless of their clinical presentation.
An elevated Interleukin-6 (IL6) is an indicator of their cytokine storm. If this is elevated watch these patients closely with both eyes.
Other factors that appear to be predictive of poor outcomes are thrombocytopenia and LFTs 5x upper limit of normal.
Disposition I had never discharged multifocal pneumonia before. Now I personally do it 12-15 times a shift. 2 weeks ago we were admitting anyone who needed supplemental oxygen. Now we are discharging with oxygen if the patient is comfortable and oxygenating above 92% on nasal cannula. We have contracted with a company that sends a paramedic to their home twice daily to check on them and record a pulse ox. We know many of these patients will bounce back but if it saves a bed for a day we have accomplished something. Obviously we are fearful some won’t make it back.
We are a small community hospital. Our 22 bed ICU and now a 4 bed Endoscopy suite are all Covid 19. All of these patients are intubated except one. 75% of our floor beds have been cohorted into covid 19 wards and are full. We are averaging 4 rescue intubations a day on the floor. We now have 9 vented patients in our ER transferred down from the floor after intubation.
Luckily we are part of a larger hospital group. Our main teaching hospital repurposed space to open 50 new Covid 19 ICU beds this past Sunday so these numbers are with significant decompression. Today those 50 beds are full. They are opening 30 more by Friday. But even with the “lockdown”, our AI models are expecting a 200-400% increase in covid 19 patients by 4/4/2020.
Treatment Supportive
worldwide 86% of covid 19 patients that go on a vent die. Seattle reporting 70%. Our hospital has had 5 deaths and one patient who was extubated. Extubation happens on day 10 per the Chinese and day 11 per Seattle.
Plaquenil which has weak ACE2 blockade doesn’t appear to be a savior of any kind in our patient population. Theoretically, it may have some prophylactic properties but so far it is difficult to see the benefit to our hospitalized patients, but we are using it and the studies will tell. With Plaquenil’s potential QT prolongation and liver toxic effects (both particularly problematic in covid 19 patients), I am not longer selectively prescribing this medication as I stated on a previous post.
We are also using Azithromycin, but are intermittently running out of IV.
Do not give these patient’s standard sepsis fluid resuscitation. Be very judicious with the fluids as it hastens their respiratory decompensation. Outside the DKA and renal failure dehydration, leave them dry.
Proning vented patients significantly helps oxygenation. Even self proning the ones on nasal cannula helps.
Vent settings- Usual ARDS stuff, low volume, permissive hypercapnia, etc. Except for Peep of 5 will not do. Start at 14 and you may go up to 25 if needed.
Do not use Bipap- it does not work well and is a significant exposure risk with high levels of aerosolized virus to you and your staff. Even after a cough or sneeze this virus can aerosolize up to 3 hours.
The same goes for nebulizer treatments. Use MDI. you can give 8-10 puffs at one time of an albuterol MDI. Use only if wheezing which isn’t often with covid 19. If you have to give a nebulizer must be in a negative pressure room; and if you can, instruct the patient on how to start it after you leave the room.
Do not use steroids, it makes this worse. Push out to your urgent cares to stop their usual practice of steroid shots for their URI/bronchitis.
We are currently out of Versed, Fentanyl, and intermittently Propofol. Get the dosing of Precedex and Nimbex back in your heads.
One of my colleagues who is a 31 yo old female who graduated residency last may with no health problems and normal BMI is out with the symptoms and an SaO2 of 92%. She will be the first of many.
I PPE best I have. I do wear a MaxAir PAPR the entire shift. I do not take it off to eat or drink during the shift. I undress in the garage and go straight to the shower. My wife and kids fled to her parents outside Hattiesburg. The stress and exposure at work coupled with the isolation at home is trying. But everyone is going through something right now. Everyone is scared; patients and employees. But we are the leaders of that emergency room. Be nice to your nurses and staff. Show by example how to tackle this crisis head on. Good luck to us all.”

RT would know if patients are sent home on oxygen, but wouldn’t particularly know about what meds are sent. And spent a lot of time on vent settings, bipap, MDI, etc. Easy enough to read labs from the admitted patient charts.

If the person’s intent was to wrongfully degrade the efficacy of HCQ, he would have done a much better job than merely saying he hasn’t found it useful, and that the hospital is still using it, and that the studies will tell.
The piece reads to me like an open, honest opinion, recognizing that the hospital is not going along with him and that time will tell. In fact, time will probably very shortly tell.

A cousin’s mother in law was buried this week and only immediate family was there. They were pretty devastated, but MIL was a nurse, so they knew she would understand.
Given the numbers in this part of the country, I’m not sure I understand. This is a metro of 2.8-3 million people depending on where the line is drawn and there are about 300 cases spread out among 17 city/county entities. That’s about one tenth of one percent.

As virus cases surge, Tokyo governor says lockdown may be only way to stem rise

Tokyo Gov. Yuriko Koike said Monday that a citywide lockdown may become “the only option left” if various measures fail to prevent an explosive rise in the novel coronavirus in the capital.
“Cities around the world are in lockdown,” Koike said during a news conference Monday. “In Tokyo there is serious concern that clusters of infected individuals will lead to an explosive increase in the number of cases in the city. If that were to happen, we may have no choice but to impose strict isolation measures — a so-called lockdown — to prevent further spread.”
Despite ongoing measures to promote social distancing, as well as school closures and the cancellation of major public events, Tokyo was described as a region “at significant risk of further outbreaks” during a meeting of the government’s expert panel last week. According to an estimate by the health ministry, Tokyo could see upwards of 530 positive cases by April 8.
An additional 17 individuals were confirmed on Tuesday to have tested positive for the novel coronavirus, the largest number of cases the city has seen in a single day — breaking the record set just a day earlier.
As of Monday, Tokyo has reported 154 cases of infection and four deaths. Of those that tested positive, 101 were hospitalized, 31 of whom have subsequently been discharged. Koike warned that the next three weeks are critical, as Tokyo could see an “overshoot,” or an explosive increase in cases.
“It goes without saying that Tokyo is the biggest city in Japan and serves as a gathering place for businesses, universities and people from around the world,” the governor said. “Because of this, the city is at great risk of experiencing clusters of infection emerging among youth.”
But it’s impossible to decipher whether the governor is truly considering a citywide lockdown, or simply mentioning such a possibility to frighten people into complying with efforts to prevent the virus from spreading further, said urban planning expert Hiroo Ichikawa.
“Shutting down Tokyo would be a massive ordeal,” he said. “The question is now whether Koike means it and specifically what she has in mind, or if she’s using it as a warning to scare people into taking the pandemic seriously.”
Several cities in northern Italy, parts of New York City and London, as well as countless other places around the world, are experiencing varying degrees of restrictive measures in order to prevent the virus from further spreading. On Tuesday, China lifted its lockdown on Wuhan, the city thought to be the origin of the novel coronavirus, nearly four months after the first cases were reported.
A lockdown in Tokyo would most likely come in phases and begin with the public transportation system, Ichikawa said. While Tokyo Metro and the East Japan Railway Co. are privately owned, they would surely heed Koike’s request if she were to call on them to scale back or shut down traffic, he said.
This would be followed by the prohibition of mass gatherings, Ichikawa went on.
Italy quickly placed several towns under lockdown, going as far as to fine or imprison violators and deploy the military to stand guard. The question arises whether the Tokyo Metropolitan Government or the national government have the authority, or the ability, to enforce such strict rules.
The short answer is no, according to Koju Nagai, a lawyer based in Hyogo Prefecture.
While municipal leaders can request or demand in varying degrees that residents stay indoors or schools temporarily close, they can’t enforce penalties on those who refuse to comply, Nagai said.
Over the weekend, despite Tokyo’s warnings against it, massive crowds gathered in local parks to see the cherry blossoms in full bloom. Countless public events have also been canceled in a bid to avoid mass gatherings during a festive time of the year when young adults graduate from school and start their first jobs.
“It’s unclear if Koike is envisioning a lockdown of the city’s functions or the movement of people,” Ichikawa said. “Hopefully she’ll specify what she meant, and what such an unprecedented decision might entail, in the coming days.”

Ground report: Nash region…highways have Very light traffic. Many more empty parking lots than last time i drove into town. Good – TN is staying home and slowing the spread. Closed schools in infected areas Very Quickly and rest of state followed suit relatively quickly. I think that helped.
PS: wonder if I post longer because of posting almost exclusively from lap top v. typing into phone? And that I learned typing (v. “keyboarding”) back in the day (remember having to correct multiple sheets of paper when typing in duplicate or triplicate?! This is good news…Hospital through put is a major issue and strategy.
Everyone is being pushed to the next farthest level to make room for incoming ICU WuFlu patients. Feds have rolled back red tape and regulations specifically for this.
Patients that would normally stay in the hospital are being pushed out early to the skilled care and patients that would normally be in skilled care are being pushed to home/self care that are dependent on visits from home health care workers/nurses.
One of the many challenges with this plan is the sudden surge in demand of patients at home and need for home health care workers and nurses to do the home visits and check on the patients.
This is a solution to help facilitate changes of standards who are going to be at home that might have been in skilled care in normal times.
this is also a working solution to free nurses to be in the hospitals giving skilled care to both meet the increasing demand for nurses as patients surge + help to cover for the increasing number of nurses who are sick themselves with WuFlu. Shortages in nurses and medical care due to increasing medical workers having Wuflu and increasing WuFlu patients.
So glad so much of fed gov and private industry including healthy care companies are forward thinking!

Based on my own experience, if you THINK you might have it, it changes behavior, and reduces the chance of spousal transmission. I didn’t think I had it, so my wife kept in contact and she got it. She MAY have gotten it before I became symptomatic, but not sure. More likely AFTER I became symptomatic, because she didn’t separate herself from me.

Ok I have kinda held my tongue about Gov Cuomo (well to some extent). I have seen the PCs, I have read as much as I can about what is going on in NYC and I want to offer my point of view. Feel free to disagree, offer additional points ..whatever. I am not an expert just someone interpreting what is going on.
This guy (or guys..DeBlasio too) is messing up completely. In a really bad way and it is affecting the country. Why?
The governor cannot tell you what each hospital needs. He has anecdotal information, He uses projections based on positive testing. He doesn’t even know where his supplies are. Pres Trump embarassed him by providing a photo of ventilators delivered to a warehouse in NJ.
He should be getting/giving a rundown of what each hospital needs on a daily basis. All of them, Every one. Tell us one by one. Then his people should be matching up that with their actual supply chain. The PPEs can be managed effectively with a precise focus on inventory. The ventilators are a different issue. He can’t possibly handle 30,000 ventilators. Not doable. Not enough trained personnel to deal with it. Plus the needs vary from hospital to hospital. That is a panic-stricken projection that they have not thought through.
We need to give him what he needs in a timely way but not dump it on his lap all at once. That jeopardizes the needs of other states. Now Pres Trump is marshalling as many resources as he can get. Even dropped the hammer on that Dem lady who runs GM. But Cuomo/DeBlasio are harming the response that the Feds can give other states.
Cuomo needs to alter his approach to become more granular in his assessments for the public. He needs a superior supply chain management guy. Top notch. Someone who can walk in and help NYC out ASAP. He and DeBlasio need to assign state/local people in every single hospital in the city so they can report accurately. Perhaps cases can be tranferred to other hospitals (probably already thought of but hey).
This blame game is getting tiresome and it is affected the needs of other cities by diverting Federal attention to solving their problems for them.
Basically Cuomo and DeBlasio are in waaaayyy over their heads and they are not smart enough to organize their actions to address their needs effectively in a timely way. I haven’t even touched on their policy decisions or the statements of the people they hired. Their liberal inclinations are biting their ass now and they hired incompetent people. It is affecting the entire nation now.
OK just some thoughts. Fire away. I am sure I got some things wrong but I am frustrated. I am in NY and it is changing what we do even here in upstate NY. I don’t see an end yet. Go easy..I am a novice at this.

From what I’ve read, voter turnout in NYC is so low it is why people like AOC can get elected with something like 16,000 voters….and it’s equally bad for important offices like Mayor. And, that’s not even addressing the corruption and fraud of the unions.
So, in some respects if you elect incompetent politicians who are in it for the money how an you expect them to actually run a city in a crisis? It’s a parallel situation to the hurricane in Puerto Rico.

According to worldometers, the U.S. now has 2,463 serious and critical cases of the WuFlu in the country. If I remember reading correctly, you would be classified as serious if you need supplemental oxygen. About 1/3 of the total serious and critical cases need mechanical ventilation. That would be 821 cases that need mechanical ventilation in the U.S.
New York currently has about 43% of the active WuFlu cases in the country. Assuming an even distribution of the cases across the country, then New York currently needs about 343 ventilators. Naturally they need to have a safety margin, and the number of ventilators needed by NY is growing rapidly.
So they don’t need an emergency drop of 30,000 to 40,000 ventilators. They have a certain number in use now, and 2,000 more should keep them going for, maybe, 12 days at the present growth rate of about 15%.
That’s not much time, so more ventilators will be needed very shortly if that growth rate continues, reaching 30,000 ventilators in about 32 days . . . again, if that growth rate continues and ignoring any ventilators that may be freed up by the recovery or death of the user.
The bottom line is that if the WuFlu continues to grow across the country at anything near a 15% growth rate, we will, indeed, needs lots of ventilators quickly.

Until supply exceeds medical staff’s extended needs the sick are going to be denied? Question then becomes who is medical staff and what more is being hoarded for whom? Example does some mayor or city official want it for his police, fire, ambulance, personal staff or personal use? Where would it stop. The right answer should be it goes straight to the sick and only the sick, not the maybe sick.
If this type of hoarding is suspected then they should radio tag the packaging and see where it’s going (sitting actually, held in reserve at someones home?)
